Our Model of Care

We’ve been where you are. Our seasoned team of Care Managers personally knows what it is like to be surprised by the unexpected when it comes to parenting. Based on input from decades of lived experience combined with expertise in best-in-class trauma-informed training, our Model of Care will put your family on a path of healing. From the moment you speak with our Intake Specialist, you will be heard with compassion and without judgment. You’ll be assigned a Care Manger who will listen to the challenges you are facing. Based on your concerns and our observations, you will receive a week-by-week plan for restoration in your family. Your Care Manager will walk alongside of you, guiding and encouraging you along the way.

You choose the frequency and duration of your scheduled accountability-driven sessions that result in stronger connections to your children.

Therapeutic Resources

Chosen matches parents with evidence based, therapeutic services and resources that meet the needs of their children.

Crisis Support

If a difficult event occurs outside of your scheduled parent coaching session, our Care Managers are available to help.

Children and teens with hard histories need a different kind of parenting and our coaching sessions include best-in-class teaching.

Clinical Assessments

All families complete and receive measurements of parental stress and trauma-related behaviors, which help us construct your customized plan for healing.

We create weekly to-do’s that will bring you and your family closer together. To-Do’s range from self-care exercises to scheduling appointments, etc.

We base our fees on family size and household income. We believe our services should be available to all. Finance should never hinder a family from healing.

Meet Our Intake Specialist

Erika Pfluke is a Care Manager and Intake Specialist at Chosen, she will be the first person your family connects with and her empathy is incredible.  Erika holds a bachelor’s degree in English from the University of Texas at San Antonio and is a Trauma Competent Caregiver trainer. Erika has been involved in the adoption community since 2009 when she and her husband began the process to adopt their daughter from China. Their eyes were opened to the plight of orphans moving them to travel to China two more times to bring home another daughter and a son. Coming alongside adoptive families is her great joy, and she is passionate about equipping and encouraging others for this journey.
